POSITION OVERVIEW
Harbor Health is looking for knowledgeable Health Coaches to become members of our team. Harbor Health is an entirely new multi-specialty clinic group in Austin, TX utilizing a modern approach to co-create health with those who get, give, and pay for it, allowing everyone to fully flourish. Join us as we build a fully integrated system that connects care to a better payment model that truly puts the human being at the center.
Health Coaches will be an important member of the care delivery team. They will work directly with clients to understand what health means to them and how our organization can best support them in achieving that vision. Health Coaches will work in lockstep with clinical providers to navigate through care pathways, they will identify non-clinical support services in the community that might help a client achieve their health goals, and they will generally steward a client’s well-being across changing health phases throughout their lifetime. A Coach’s primary responsibility will be to make it as easy as possible for the client to stay informed and maintain agency in the co-creation of their definition of optimal wellness.
POSITION DUTIES & RESPONSIBILITIES
Health Coaches are responsible for:
- Welcome members to the organization’s unique model of health delivery and get to know their personal health journey, experiences, preferences, and orientation.
- Helping members set personal health goals and orient to the data, tools and care team that will support them in achieving these goals, as well as conducting light assessments or screenings to inform care pathways.
- Educating clients about the Coming Soon Health pathway design and member decision-making. Answer questions and support members in taking the next step and assessing barriers to access or challenges to pathway completion and mitigating accordingly.
- Tracking client events/activities along their care pathway and follow up with providers to ensure next steps are completed.
- Building trusting relationships with Coming Soon Health’s primary care team as well as network care providers to ensure care coordination and communication are strong across all care teams. As necessary, escalating, and de-escalating client needs between clinical and non-clinical services.
- Receiving client questions, suggestions, and complaints with grace and empathy, while demonstrating listening through action by following up with information, next steps, or resolution in a timely manner. Where appropriate, supporting medical education or identifying appropriate sources for support and determining with the team how to build client inquiries into a sustainable service model.
- Building a trusting relationship with the client to follow them across their life cycle. Following-up, checking-in, and reaching out to ensure the client's health journey is aligned to their life cycle needs.
- Leveraging the organization’s technology platforms to aid the client in partnering on the creation of their own health and wellness vision.
- Documenting important information from client conversations to support care delivery, personalization, and process improvement.
DESIRED PROFESSIONAL SKILLS & EXPERIENCE
- 1 year of experience in a customer-facing service role
- High School Diploma or GED
- Physical ability to sit, stand, and walk; this role may include a combination of seated computer/phone work, onsite and in-person client interactions, and public events such as health fairs, farmers markets, etc.
- Tolerance for car travel; our clinic offerings include mobile units that travel to clients’ neighborhoods, requiring up to about 3 hours of driving either in a personal vehicle or in a company vehicle with the care team.
- 1 year of experience in a healthcare role (e.g., social worker, community health worker, care manager, etc.)
- 1 year of experience in a coaching role (e.g., health coach, life coach, weight loss coach)
- 1 year of experience in a holistic wellness specialty (e.g., trainer, nutritionist, SDoH, etc.)
- BA, LCSW or other wellness or coaching certificates
- Bilingual
- Medical translator
